Here is the whole thing, done. Ten seconds of setup before class, short lines during it, questions and a summary afterwards. Perhaps four minutes of work on top of the class you were sitting in anyway.

Notice the last note line: a question mark and a thing to ask. Writing down what you did not follow is as valuable as writing down what you did, and it is the line a tutor can do the most with.
